Denis is a member of the Institute of Clinical Psychologists (ICP), the Australian Psychological Society (APS) and several other professional psychology associations (see Biography). (All ICP members have a minimum of 8 years training in psychology). He has served on the Management Committee of the Anxiety Disorders Foundation, and the Perth West Domestic Violence Action Group, Inc.

Medicare announcement
The 9 October 2006 Medicare announcement of new psychology items heralds new era for mental health services in Australia.
“The reforms are a significant recognition of the effectiveness of psychological treatments delivered by psychologists, representing a major milestone for mental health and the profession of psychology," said Australian Psychological Society (APS) President, Amanda Gordon.
The Medicare initiative has been designed to encourage team-based mental health care in the community, with psychologists working alongside GPs, psychiatrists, mental health nurses and other allied mental health professionals.
Key components of the Government's announcement include:• Psychology services under Medicare can be provided to people with 'an assessed mental disorder' that is being managed by a GP under a GP Mental Health Care Plan or a psychiatrist assessment and management plan, or on direct referral from a psychiatrist or a paediatrician (for treatment of a child)
• Psychology services under Medicare are limited to a maximum of 12 individual sessions per client per calendar year, with a review to the referring doctor required after the initial six sessions. In addition to these 12 individual psychology services, clients will also be able to claim up to 12 group session psychology services, where appropriate, in a calendar year.
• Clients cannot use private health insurance ancillary cover to 'top up' the Medicare rebate paid for psychologist services. They need to decide if they will use Medicare rebates or their private health ancillary insurance cover to pay for psychologist services.
• Registered psychologists who are eligible to provide the clinical psychology items can charge the scheduled fee and either bulk bill or add on a gap fee. Most Clinical Psychologists charge a gap fee. Please refer to the Medicare Benefits Schedule (MBS) items for psychological services tabled below. Often 6 or less sessions is enough to deal with a crisis or concern. Under Medicare up to 12 sessions can be provided to eligible clients. Please check with your GP.

The consultation fees that psychologists charge vary, and depend on the type of service being offered. The Australian Psychological Society has a Schedule of Recommended Fees for private practice psychological services. However, APS Psychologists may adjust fees according to the client's financial circumstances.

Fees
Business & Management Fees:
- Standard rate: $250 - $350 (onsite)/ hour, or a negotiated day rate.
- Training programme fees vary, ranging from $2,000 per day
- Employee Assistance programme fees are closer to Private Clinical Psychology fee rates, by negotiation.
- Dispute Mediation fees are $250 / hour with a 4-hour minimum Mediation-session plus pre- and post-session fees as required.
Clinical Psychology Fees & Rebates:
Clinical Psychology fees vary according to the client and nature of the work:
- Private clients - who pay from their own funds and who may have private insurance to cover part or all of the fee. Our services are rebated by all funds. Rebates vary between funds. Contact your health fund for up-to-date rebate rates.
- Medicare patients - referred by a GP under a ‘GP Mental Health Care Plan.
- Workcover clients - the Worker's Compensation Scheme pays all of the fee.
- E.A.P. employees - fees get paid by their employer's E.A.P. funds, as above
- Motor vehicle accident clients - fees get paid by ICWA (www.icwa.wa.gov.au) Contact ICWA and a Personal Injury Lawyer specializing in M.V. compensation and claims arising from road accidents to clarify your entitlements and cover.
Category 1 or 2 clients pay at the time of consultation. Payment can be by cash, cheque, VISA, AmEx or Mastercard. When payment is not made by the end of each session the APS recommended fee (AS ABOVE) plus a $20 accounti service fee may be applied. Concessional rates may be negotiated. Please note that health services are GST free.
Category 3, 4 or 5 clients do not pay at the time of consultation. The APS schedule of fees apply (see above table). An invoice is sent to the employee or fund who has agrreed to pay for the service provided. Documentary (written) evidence that this is so must be provided before the service is provided.

Changed Appointments : If you want to change or cancel an appointment, two full working days (48 hours) notice is required. Occasionally people are prevented from keeping an appointment because of sudden illness or unexpected emergencies. If this happens to you, please contact the clinic as soon as possible, and the fee will be waived.
Broken Appointments : If you do not give notice of your inability to attend before your scheduled time, your nonarrival will be treated as a broken appointment, and the full session fee will apply..

Private Clinical Clients
Private insurance
Many insurers now rebate 30-80% of psychological services.
If you have private health insurance and wish to claim a rebate please contact your health insurance provider to ascertain your eligibility for psychological rebates. Details vary, so please check your policy wording. If eligible, ensure you receive a receipt at the end of each session for which you wish to claim. At each session’s end you will be charged the consult fee. You will then need to apply for a rebate from your health insurer.
Private Clinical Fees
Private fee charges are according to the APS* recommended rates for psychologists in Australia for an individual or couple earning at least a basic wage. For others there is a sliding scale of charges based upon income to ensure the service is accessible to all. In these cases, the client's future contribution is negotiated at the first meeting.
Clients referred by a GP for services under Medicare or Workcover please refer to the information below under "Third Party Clients". Health Care Card holders (HCCH) and Full-time Tertiary Students pay a $24 gap for a 30-50 minute session. Others pay a $64 gap. >> click here
After the first session, most sessions are a recommended 1.5 hours, to get you moving quickly toward your solutio (clients may however opt for 50-55 minute sessions).. Biofeedback, neurotherapy and some EAP sessions are 50-55 minutes, whereas couple and hypnotherapy sessions are usually 90 minutes.
- First session: $160/hr/ 50-55 min session (Private, non-corporate, non-Medicare) This special rate applies to your first session so that you can 'dip your toes in the water' at lower cost. It's a mutual reconnaissance, as well as an opportunity to get moving on urgent issues. You can then decide how you would like to move forward.
- Private rate: $190 / 50-55 min session = $275 for 90 min session (Private, non-Medicare)

Private '6-Packs' - available for individual consultations, couple therapy, hypnosis and other (non-EEG) biofeedback modalities, including HeartMath. Group Therapy '8-Weekly' - $480 per 8-sessions (or $270 a month), paid in advance. Group Therapy '6-Wk+1Sat ' - $700 per 8x2.5hrs (or $400 a month), paid in advance.
Neurotherapy intake: Getting started in neurotherapy usually involves a 45-50 minute initial consultation to discuss the individual's needs and whether neurofeedback might help. This is followed by a three hour testing and evaluation session that includes a sample neurofeedback session. In my practice the initial consultation costs the same as a single neurotherapy session, and the testing and evaluation costs $350-$500, depending upon tests required.
Neurotherapy 'Standard Module (10-pack)' - Neurotherapy training is a programme based on modules of 10 x 45 min sessions. The basic training programme is 3 modules. However depending on the needs of individuals a programme can be one to six modules. Three sessions a week is recommended, especially in the first few weeks of a programme, with two sessions a week being the minimum. When a client has an unusually long treatment the option of home training can be explored after stabilization and basic progress have been made.
The fee for a standard module is $1,400 and each module must be paid in advance before training begins. If 3 modules or more are paid for at once an additional 10% discount applies.
Neurotherapy insurance cover: Insurance companies vary in their willingness to cover the cost of neurotherapy. There are certain diagnoses for which biofeedback has become an accepted treatment method, and since neurofeedback is a form of biofeedback (EEG biofeedback) it will usually be covered in those situations. The treatment of migraines is one example of this. You might want to ask your insurance company if biofeedback is covered for your particular problem, and if so, whether it is considered to be a medical expense or a mental health expense.

Medicare rebates for accessing psychologists are only available following an assessment by your G.P., psychiatrist or paediatrician to determine eligibility. It is advisable to request a long consult with your G.P. as a thorough assessment is required. The medical practitioner will determine your eligibility and may specify the number of sessions you may access.
Typically people are referred for up to six sessions. If further sessions are required, the psychologist will consult with the referring medical practitioner to request this. A total of 12 sessions per calendar year are allowed under the medicare scheme.
Healthcare Card holders (HCCH) referred by a GP under a Medicare funded ‘GP Mental Health Care Plan’ only pay a small gap fee*. However, to maintain this net low-cost service to patients who attend appointments, cancellation fees may apply in the instance of patients making late cancellations or failing to keep appointments. The cancellation fee is not claimable under Medicare and must be met in full by the
patient. Read the section on "Broken Appointments" further below.

To calculate payment due at the end of each session add the "Rebate" (rounded to the dollar) to the "Gap fee". For example, on an item 80010 the full (non-concessional) fee is the sum of $112 and $73, which is $185.00 for a 45-60 minute session (full fee for a 61-75 min consultation is $230 and a 76-90 min consult $270 ). Once you've paid you can take your receipt to Medicare who will then pay you the Rebate.

Workcover (Worker's Compensation Scheme)
For full payment of psychology sessions by Workcover WA ( the statutory authority responsible for the regulation and administration of the workers' compensation system in Western Australia)., clients need a referral from their GP. At the first session, Workcover clients need to provide their current Workcover claim number, the date of their work related injury, and employer and insurance company names and contact persons.
Please note, Workcover pays only for the costs of psychological services directly related to the treatment and management of a work-related injury. It does not provide payment for other issues affecting clients (e.g., early life issues, current family problems).

Credit card payments incurr a cost ("merchant service fee").
" Merchant service fees are a significant cost to small business.
Merchant service fees are those imposed by the merchant's bank each time the merchant accepts a credit card payment. Merchant service fees are calculated as a percentage of the value of the goods and/or services purchased. ' -
Australian Competition and Consumer Commission.
In August 2002 the Reserve Bank of Australia released three credit card related reforms. One of these removed credit card rules that prevented merchants from recouping the cost of merchant service fees directly from consumers who purchased goods or services using a credit card. This means that from 1 January 2003 consumers could be charged an additional fee when they purchase goods or services with their credit card.
Private practitioners typically pay their bank 5.5% on deposited credit card payments. That amounts to $5.50 out of a $100 fee payment ($8.25 / $150, $14.0 / $255). In his practice Denis McCarthy has decided it is fair that this cost is shared 50:50. In other words, all clients will be charged a 2.75% surcharge on credit card payments, i.e.. $2.75 on a $100 fee, $4.13 on $150, $5.25 on $190, etc.

Info About Appointments
All information and records regarding you are kept strictly confidential.
Appointments are for 50 to 90 minutes (usually 1.5 hrs) each week or fortnightly. Most sessions are 90 minutes to get you moving more quickly toward your solution. Couple therapy, group therapy and hypnosis sessions are generally 90 minutes. Neurotherapy or EEG biofeeback sessions are 50 minutes, and preferably twice weekly.
Most people do not need a large number of consultations (the average is 6 sessions). Neurotherapy is an exception to this. Although EEG biofeeback often produces noticeable changes in 6 sessions, at least 20 sessions are needed to consolidate benefits. Similarly, individuals in Denis's therapy groups often repeat a group to consolidate gains and further their growth.

What happens on my first visit to a Clinical Psychologist?
During your first appointment(s), your clinical psychologist usually asks you about why you have come to see him or her, and to describe your problem in as much detail as possible. The Clinical Psychologist's questions will likely include: when did the problem start, under what circumstances does it occur, how long does it last, has it occurred before, and what makes it worse or better. The clinical psychologist may also ask questions about how the problem has affected your life in terms of personal distress, interpersonal relationships, family relationships, social relationships, employment, financial distress, recreation, and so on. As background in exploring your current problem the clinical psychologist may also take a detailed personal and family history. Similarly, the psychologist may also ask about your medical history, medication use, and use of alcohol or drugs. The information-gathering process is designed to provide the clinical psychologist with a comprehensive evaluation of the problem and its effects of your well-being. This information-gathering phase can take one or more sessions and may be supplemented by the use of psychological tests. Finally, the clinical psychologist will work with you to determine how he or she can help you manage or overcome your problem and its impact on your life.
What is involved in a Psychological Assessment?
Depending on the problem, a psychological assessment may go beyond the initial intake interview to include psychological tests or questionnaires. If you, your child, or your family is having an assessment done, the clinical psychologist will advise you as to how much time your particular assessment is likely to take. It could be as little as a couple of hours, or could take place over two or more days, depending on the complexity of the issue and the number of people involved in the assessment.
A psychological assessment is not a case of "one assessment package" fits all. The experience of the clinical psychologists informs the choice of psychological tests that are most suited to understanding your problems. There are a number of different psychological tests and questionnaires that are designed to explore specific aspects of human experience such as mood, personality characteristics, concentration and memory, alcohol and substance use, anger, relationship problems, coping, parenting, and educational strengths and weaknesses, to name a few. Once testing is complete, the clinical psychologist combines information gathered from interviews, tests, questionnaires, and non-verbal cues to arrive at an impression or diagnosis of your particular problem. Finally, the clinical psychologist reviews the results of the assessment with you and discussed how the assessment is used in the development of a treatment plan.
What is involved in Psychological Treatment?
During treatment sessions, your clinical psychologist works to help you gain a greater understanding of your problem and provide you with awareness, knowledge, skills, and/or recommendations to manage or solve your problem. Specifically, your clinical psychologist assists you in exploring your problem-related thoughts, feelings, and behaviours. During this process, you will be encouraged to develop your own set of therapy goals and methods of successfully achieving these goals. There is more than one way to challenge a psychological problem. The special procedures or techniques chosen by your clinical psychologist will depend on the psychologist's training and theoretical orientation and the characteristics of you and your presenting problem.
In terms of the practical aspects of psychological treatment, a session with a clinical psychologist typically lasts about 50 minutes, but may vary depending on the individual psychologist, your needs, and the kind of activity being done. Psychological treatment may take place over a very short period of time or over a lengthy period of time depending on your individual needs and preferences and the problem being addressed.
Similarly, psychological treatment may vary in format. The most common forms of treatment include individual, couples, family, and group therapy. Couples and Family Therapy formats take into account the benefit of everyone affected by the problem present in the session. This is especially helpful in identifying and working on maladaptive communication and role patterns. The benefits and drawbacks of individual versus group therapy are more complicated. Whether individual or group therapy is better depends more on the person, the problem, and format availability. You may want to consult with a clinical psychologist regarding which format is best for you.

New psychology Medicare items
The mental health psychology Medicare items involve two categories - 'general' psychology services and 'specialist' clinical psychology services.
Both general and specialist psychology Medicare items can only be provided to people with 'an assessed mental disorder' that is being managed by a GP under a GP Mental Health Care Plan or a psychiatrist assessment and management plan, or on direct referral from a psychiatrist or a paediatrician (for treatment of a child).
All psychology Medicare services are limited to a maximum of 12 individual sessions per client per calendar year, with a review by the referring doctor required after the initial six sessions. In addition, clients will also be eligible for 12 group session services, where appropriate, in a calendar year.

Services should be provided, as required, in up to two sets of six sessions, with the need for the second set of sessions to be reviewed by the referring medical practitioner after the initial six sessions.
